Was the build of Geant4 itself successful - i.e. does the log file show any errors? The reference problems are due to Qt rather than OpenGL and in the interfaces library rather than the visualization libraries.

That suggests a problem with the interfaces library, or, had you built the example against an older version of Geant4 (i.e. you had old object files sitting around)? If the latter, then a make clean && make should do the trick. Given that it doesn't complain about libG4interfaces being missing, I'd suspect this library was built o.k.
