I ran example extended/runAndEvent/RE02 and obtained the attached. There's no sign of a ripple.

RE02 with no lead in phantom; 420*420*300 mm3 phantom; 21*21*300 voxels; QGS_BIC_EMZ physics (i.e., EM option 4); 190 MeV protons, pencil beam, 1e6 primaries; development version of Geant4 (probably close to 10.5)
