You are making a new empty G4IonTable. What you want is G4's G4IonTable:

#include "G4IonTable.hh"

 G4ParticleDefinition* ion = G4IonTable::GetIonTable()->GetIon(Z,A,excitEnergy);

As for the deuteron, you can also use:

G4Deuteron::DeuteronDefinition()

if it's made in your PhysicsList

It's probably good to also set:

theParticleChange.SetNumberOfSecondaries(2)
