 Did you check that rdecay01 works on your installation ? (for example with commands like:

 /gun/particle ion
 /gun/ion 95 241
 #
 /rdecay01/fullChain true
 #  
 /run/beamOn 1

  If not, have you correct data sets and environment variables ?
    setenv G4LEVELGAMMADATA "/home/maire/g4data/PhotonEvaporation2.2"
    setenv G4RADIOACTIVEDATA "/home/maire/g4data/RadioactiveDecay3.4"

  If yes, we need to see your physics list and primaryGeneratorAction !
