At first, reading your message, I thought that maybe the G4BoundaryProcess can't handle this and is not protected for the eventuality that two dielectric materials have exactly the same index of refraction. But that's NOT the case! I tested it and all works well within G4OpBoundaryProcess.

Something else is not right in your geometry definition. You could test this by giving one of the media a slightly different index (1.01 if you like) and see what happens. Also, please set

/control/verbose 2 /tracking/verbose 2 /process/verbose 2

and include the print out in your reply.

I now suspect that you didn't define the optical properties of your materials (including the vacuum) as shown in exampleN06? If you didn't G4OpBoundaryProcess bails out by killing/stopping the track.
