The meaning of "efficiency" simply rely on your definition
with respect to your application. 

If you want to see the "detection efficiency" of your 
detector made of BGO and presumably a photo-multiplier, 
then you have to generate optical photons in your BGO 
crystal and trace them to your photo-multiplier. Please 
note that Geant4 cannot simulate "quantum efficiency" of 
your photo-multiplier.

If you want to see the ratio of primary gammas which
pass through the measure, then you don't even generate
EM shower in your BGO, but just count the number of gammas
which arrive to the surface of BGO.
