Yes, this has been seen before. The radioactive decay proceeds correctly, but the subsequent relaxation of the atomic shells is ignored for some reason. It has something to do with the interface between G4RadioactiveDecay and G4AtomicRelaxation, which I thought had been fixed a while ago.
