I would strongly recommend replace Physics List you have shown by another solution because you have mixed two approaches for the Physics List together and results are hardly be predictable. There are different possibilities:
   1) take directly QGSP_BERT_HP 
   2) use G4PhysicsListFactory - example is $G4INSTALL/examples/extended/hadronic/Hadr00
   3) the most complicate is to follow $G4INSTALL/examples/extended/hadronic/Hadr01

You may choose one but not mixture.

Independently of that you can check your installation of Geant4 if you use Hadr00 without any change, define environment variable PHYSLIST=QGSP_BERT_HP, your materials of interest and primary neutrons. If you will get the same exception than installation of Genat4 has a problem or you do not have a path to G4NDL data library.
