For most cases, the hElastic process uses the G4LElastic hadronic model which does not produce nuclear recoils if the incident momentum is less than 10 MeV/c.

You could try instead the G4VHadronElastic model which has no such limit (although you can set one if you wish). In that case you would always get a recoil nucleus.
