See the related Hits thread. Your implementation of
VoxelPhantomParameterisation::ComputeTransformation()
method is incorrect.

The reason why you could visualize your phantom successfully
is that your ComputeTransformation() method is invoked with
incremental CopyNo for visualization (i.e. voxels are visualized
one by one sequentially). On the other hand for the real tracking,
CopyNo is quite random. Given your ComputeTransformation() method 
returns the incorrect position, an infinite loop may happen.
