I tested with 10.01.p02, by just adding the ntuple and it is filled ok when running the TestEm5.in up to the first /run/beamOn call.

The problem is probably in your code. I suggest you to add a printing in EventAction::::EndOfEventAction() to see that your function is called during event processing. You can also set a higher verbose level to G4AnalysisManager and check in the output that the ntuple is filled:

 G4AnalysisManager::Instance()->SetVerboseLevel(4);
