    You should use G4StepPoint::GetMomentumDirection( ) of pre-step and post-step. 
    Do not use G4Track::GetMomentumDirection( ) in this case.

   I think following questions are byeond a scope of Geant4 forum,
or it is rather ambiguous to understand these results.
  Geant4 calculates these quantities with respect to physics processes,
and those are strongly follow your simulated conditions, i.e. geometry,
physics list etc. 
   Please check your simulation condition again. 
   I suggest that it may be good to confirm these behavior step by step.
For example by simulating more simple case such as  simulating 
without water phantom,  with only simple Rohacell plate geometry, etc.
