To add a detector-oriented energy fluctuation, you should 
implement your own fluctuation function and put it in
your sensitive detector or digitizer. As Vladimir pointed
out, all "physics-oriented" energy fluctuation have already
been included in the energy deposition, while detector-
oriented fluctuations such as noise, resolution, etc., are
the items the user must take care of by him/herself.
